00. ACKNOWLEDGEMENT OF CCTV ISSUANCE AND GUIDELINES

This is to confirm and acknowledge that I, ______________________________________, have been assigned CCTV Live View Access ____________.

I have read and understand the CCTV Guidelines below:

  1. To view the CCTV Live View on the Mobile Apps, customers can submit the online CCTV booking form on the My Queen website.
  2. Upon successfully approved the submitted form by our My Queen Administrator, we will request you to share the QR Code from your Hik-Connect Mobile Apps to authorize.
  3. Refer to FAQ for Hik-Connect Mobile Apps installation and how to get the QR Code from there.
  4. Customers can submit again the online CCTV booking form if they need to have CCTV Live View on multiple Mobile Devices. However, it is subject to My Queen Admin to approve or disapprove.
  5. Service will not apply for the CCTV quality issues raised due to the customer’s own internet facilities.
  6. The Customer also agrees not to permit any third-party apps to tamper with the CCTV Monitoring Apps without the written consent of My Queen Admin.
  7. If there is a change of Mobile Device or any problems with the Mobile device, the customers should notify My Queen admin so that admin can disable the access on the affected Mobile device and help to authorize the CCTV Live view on the new Mobile Device.
  8. We shall grant CCTV Live view access based on the submitted requisition form.
  9. Customer will have Only CCTV LIVE View Access during the active period with My Queen.

By signing the below, I acknowledged the issuance of the CCTV Live View and will comply with the CCTV Guidelines above.
