To receive feedback from your learners upon course completion, one way is to create a survey and ask them to participate in answering the survey.

Here’s how to create a survey in a few steps:

1. Log in to your LOOP LMS account as an Instructor/Site Administrator. Go to Courses > Survey (1).



2. Click on "New Survey" (2).



3. Name the survey (3) and describe the purpose of the survey (4).



4. Add Survey Question (5), fill in the survey question (6) and select the question type (7). Then, save (8) and submit the question (9).
